Damage Authorization Form

I (client) authorize The Legacy Barn, LLC to charge my bank account or credit card indicated below for costs, fees, damages, or liabilities per the terms and conditions set forth in the Event Venue Rental Agreement by and between The Legacy Barn, LLC, and Client.

We will notify you of any damage or extra fees prior to the charge. Please fill out information for your Checking/Savings account OR a credit card. We require this form to be filled out because we do not collect a Security Deposit.

I understand and agree that any and all changes in my above bank account information or credit card should be sent to us prior to your scheduled event.

We will not charge your credit card or conduct an ACH withdrawal for any rental payments. Those must be made by check, cash or Venmo. However, if you make rental payments by check and there is a returned check for NSF, there will be an additional charge of $100 and we will attempt to withdraw those funds via ACH or CC. I also understand and agree that a return item charge may be assessed for each returned ACH debit. I acknowledge that the origination of ACH transactions to my account must comply with provisions of U.S. law, that I am an authorized user of this bank account/credit card and agree not to dispute charges to my bank account or credit card company so long as the transactions correspond to the terms indicated in my proposal or this authorization form.
